If you would like to go to Dublin, Ireland with us, please read, sign, date and
return this Deposit & Cancellation Policy/Sign-up Form, to me with a deposit check made payable to, Michael Baker,
in the amount of $300.00 per person. This $300.00 is your nonrefundable, nonchangeable, nontransferable deposit for the
Dublin, Ireland 2010 Trip.
A deposit is required from each person
going and is applied towards the cost of the trip. Mail your deposit along with this Deposit & Cancellation
Policy/Sign-up Form to: Michael Baker 1449 Moore Circle Corona, CA 92882.
The
cost of this trip is $1,795.00 per person based on two people sharing a room (Double/Twin).
All rates are subject to change until ticketed.
The cost of
this trip includes: #1. Round-trip air from Los Angeles International airport to Dublin, Ireland. #2. Seven nights/eight
days Hotel stay located in the heart of Dublin, Ireland. #3. A full breakfast-buffet each morning of your stay.
#4. Taxes and service charges. #5. Airport transfers from Dublin's International airport to the hotel the day of
our arrival. And airport transfers from the hotel back to the airport the day of our departure.
All totals are subject to a possible 10%
adjustment increase, based on the country tax and/or the exchange rate at the time of final payment and/or ticketing and/or
any other surcharges the airline or hotel imposes on us before final payment is made, including any fuel charges.
Once I receive your first payment ($300.00 nonrefundable, nonchangeable, nontransferable deposit),
with this filled-out Deposit & Cancellation Policy/Sign-up Form, I will then send you a statement reflecting all payments
made and any balances due.
In order to go on this trip with us, the
Deposit & Cancellation Policy/Sign-up Form MUST be signed, dated and returned to me before the final payment is made.
Failure to do so could result in cancelation of your trip.
Make all
checks payable to Michael Baker. A $300.00 Nonrefundable, nonchangeable, nontransferable Deposit is required, at
the time of sign-up, in order to go on this trip with us.
If
you cancel this trip to Dublin, Ireland on or before June 30, 2010, all monies paid, excluding the $300.00 nonrefundable,
nonchangeable, nontransferable deposit, will be returned to you no later than January 2011.
If you cancel this trip to Dublin, Ireland after June 30, 2010, all monies paid, including the
$300.00 nonrefundable, nonchangeable, nontransferable deposit, will not be returned to you. There will be no exceptions!!!
If you cancel this trip to Dublin, Ireland after the tickets have been ticketed, and have an outstanding balance,
that balance will still be considered owed. And if that balance is not paid in full before October 18, 2010,
unless other arrangements have been made, it may result in legal actions. You will be held responsible for
all legal fees from the result of legal action.
All cancellations
must be in writing.
All tickets are nonrefundable, nonchangeable
and nontransferable, period.
If for any reason, the airline and or
the Hotel cancels any of our flights and or Hotel stay, I (Michael Baker) will not be held financially or emotionally liable
for any loss incurred by this act including but not limited to any Hotel costs, transportation costs (Airport Transfers) or
optional excursions.
If you agree to the above policies, please sign,
date and return this form, with your nonrefundable deposit of $300.00 per person, to me ASAP. Failure to return this form
will mean your forfeiture of the trip and all monies paid.
